Jspace is open to any student and family who wishes to have Jewish enrichment. Are there any special circumstances regarding religious or family status (e.g. conversion, inter-faith family, adoption, single-parent family, etc.) that would be helpful for us to know about? If yes, please describe them or indicate that you would like to meet privately to discuss these with Yael. 

Please explain any specific conditions which your child has (allergies, anxieties, sensitivity to medications, etc.) that would help us assist your child in an emergency. If no specific condition exists, please write NA.
